carf clnCommunity Living Network (CLN) was created in 1997 to offer fiscal intermediary services to adults under the Choice Voucher and to children through the Children's Waiver Program. CLN is contracted with 9 local community mental health agencies to provide fiscal intermediary services across Michigan.

Why A Fiscal Intermediary?

Have you ever felt ignored or powerless with your current arrangement for personal care, job coaching, and other supports? It is your turn to make the choice. You can be in charge.

Becoming an employer is about choice and control. It is about becoming the decision-maker. It is the highest expression of the Person Centered Philosophy! It is about freedom:

  • The freedom to choose who you spend your time with.
  • The freedom of who you allow into your home.
  • The freedom to decide what activities you and your staff will participate in.
  • The freedom to control your resources.

Fiscal Intermediary Services

  • Allows individual to hire and fire their own employees.
  • The consumer decides who provides support services.
  • Adult customers make hiring decisions through the Choice Voucher system.
  • Parents or guardians of children participate through the Children's Waiver Program.
  • CLN provides payroll services assuring that all taxes are paid.
  • Customers receive a monthly statement of their financial activity.
  • Provides payroll services to ensure firewall between the recipient of service and Medicaid funds
  • The customers' employees will receive accurate pay and W2s.

Questions you should ask a Fiscal Agent

Our program services rely on strong partnerships with County CMH and the Michigan Department of Health and Human Services.  The following is the 2021 technical manual  as well as FAQs about services from the department on Self-Direction services.

  • FIS Frequently Asked Questions

    Do we need a fiscal intermediary?

    Yes, it is a requirement of the Mchigan Department of Community Health that customers wishing to hire their support staff directly utilize the services of a fiscal intermediary.

    Can I contact my fiscal intermediary directly?

    Yes, your fiscal intermediary works for you and with you, so communication is not only allowed but necessary.

    Is my personal information kept private?

    Yes, all fiscal intermediary offices must be HIPAA (Health Insurance Portability and Accountability Act of 1996) compliant.

    Can I give raises?

    Yes, as long as they are within your budget. It is often advised to start your employees at a lower rate, thus leaving room for a wage increase.

    Who get worker's compensation insurance?

    As your fiscal intermediary, CLN will help you obtain any required worker's compensation insurance for your employees.

    How do my employees get paid?

    Employer approves time sheets. Mail or fax forms to CLN. Pay periods are the 1st through the 15th and the 16th through the last day of the month. Time sheets must be received three business days after the end of the pay period. Paychecks are mailed or deposited directly into their bank accounts.

    Do you keep my employee's personnel files at your office?

    Yes, we store important forms such as IRS and MI W-4, verification of employment eligibility (I-9) and other documents.

Supported Living Program for Individuals with Disabilities


 

Community Residence Corporation (CRC) provides persons with disabilities living options, life choices, and control of their future. Guided by the commitment to focus on individual choice, CRC provides services based on the Person Centered Philosophy. Our employees are specifically trained to provide services to people with develomental disabilities to assure customer satisfaction. Our vision is to lead change in the community that celebrates and supports the lives of people with developmental disabilities.

Supported Living Program

Supportive living brings services to clients in their own homes and apartments throughout our community. Here are the key elements to our Support Living program:

  • Personal choice emphasized in every aspect of daily life.
  • Focus on developing self-sufficiency and full participation in the community.
  • Services can be adjusted to accommodate changing needs.
  • 24-hour on-call emergency support.
  • Basic fire and household safety.
  • Medication management and personal care support.
  • Accessing community resources.
  • Personal selection of staff.

Community Mental Health Partnerships

Our partnerships with Community Mental Health (CMH) agecies are critical to our success.  We have an agreed upon standard of care for Community Living Support services that we provide under our contract with CMH. All services are provided by referral from CMH.

Katherine Grant, MSW

Executive Director

Kathy graduated from Wayne State University in 2005 with a Masters in Social Work. She's been with Community Alliance since July 2011, as the Executive Director since 2013. When she graduated from college, her children gave her a plaque with an inscription that reads: I'm out to change the world and I'll start with you if I have to. That's how Kathy approaches her work on behalf of the developmentally disabled population: she is out to change the community they live in to be more inclusive. She sets her vision and standards high and inspires people to be their best.

 

Melissa Frash, BS

Director of Fiscal Intermediary Services

Melissa graduated from EMU in 1995 with a Bachelors in Science. She's been with Community Alliance since May 1996, nearly her entire adult life. She's worked in every department of the organization and is an invaluable team member. She is always playing a prank or practical joke and there is always laughter in the office instigated by her. She's a joy to work with because she understands the importance of the work we do and she's dedicated her life to it. The folks she helps everyday feel her kindness and compassion and get a dose of her humor as well.

Direct Care Staff

We have over 50 direct care staff specially trained to provide community living support services to the consumers we have the privilege to serve.

Our training protocol is approximately 8-10 full days and includes:

  • CPR/1st Aid
  • Medication Management provided through CSTS
  • Recipient Rights provided through the Office of Recipient Rights
  • Online CMH training including Limited English Proficiency , Medicaid Integrity, Bloodborne Pathogens, Cultural Diversity, Consumer Grievance and Person Centered Planning
  • 3 Day Direct Care Staff Training
  • 1 Day Site Specific Training
